POSITION OVERVIEW AND REQUIREMENTS:

Exciting opportunity in Dana-Farber/Brigham and Women's Cancer Center Ambulatory Care Coordination. Seeking a seasoned RN with case management expertise, experienced as a supervisor and manager. Direct care practice involves facilitating referrals to hospice for patients who are followed by disease-based teams. Supervisory responsibilities include management of the ambulatory RN Care Coordinator in pre-admission planning for Bone-Marrow Transplant/Stem Cell as well as managerial responsibility for other programs involving care continuum. Acts as key link between disease centers and case management resources and initiatives. Works across the continuum to ensure successful initiatives between Dana Farber Cancer Institute and Brigham and Women's Hospital. Candidate will have experience in case management, end-of-life, palliative or hospice care. Outstanding teaming, communication, planning, management and clinical judgment skills necessary.

Excellent written and oral communication skills. Masters in Nursing Administration, Healthcare Administration or Business Management. Computer skills. Reports into Dana Farber/Brigham and Women's Cancer Center Care Coordination and DFCI Ambulatory Nursing.
